- What is the assessment fee, and what does it include?
- How long is an appointment, and what portion is direct session time?
- Are written reports, preparation or meetings billed separately?
- Who provides the service, and who supervises it where relevant?
- What are the payment and cancellation terms?
- When will recommendations about frequency be reviewed?
Extended health benefits
Check directly with your benefits provider before assuming a service is covered. Ask which professional designations are eligible, whether a referral is required by the plan, whether there is an annual or per-session limit and what information must appear on a receipt.
A plan may treat services differently depending on who delivers them. Coverage for a registered professional does not automatically establish coverage for an assistant or interventionist. Direct billing is not advertised by Clover Creek unless the team confirms it.
